Access to deceased health records: William Buckler and Mary Doreen Williams

Thank you for your email of 21 August 2026 and for the application form and cover letter. I am sorry for the delay in replying. Sion will complete and sign the form himself, as the applicant under the Access to Health Records Act 1990.

Before he does, three points:
1. Corrected date. The request gave William Buckler's death as "on or about 29 May 1990", from newspaper reports. Sion has since confirmed that William Beverly Buckler died at Llandough Hospital on 15 May 1992. Please search on that date.
2. Whether records survive. Please tell us whether the Health Board still holds any records for these two patients (Mary Doreen Williams, born 10 August 1913, death registered March 1983; William Beverly Buckler, died 15 May 1992). If records have been destroyed under the retention schedule, please give the date and the schedule entry applied. This saves an application for records that no longer exist.
3. Preservation. Please make sure that any surviving records for either patient are kept while the application is made and dealt with.
