Dear Health Records Manager,

I am writing to request access to any health records held by Cardiff and
Vale University Health Board (including Llandough Hospital) relating to the
following deceased persons, under the Access to Health Records Act 1990
and/or the Freedom of Information Act 2000 where applicable:

1. William (Billy) Buckler
   - Died at Llandough Hospital on or about 29 May 1990, aged 41
   - Late of Great House Farm, Llandough
   - Reported cause of death: heart attack

2. Mary Doreen Williams
   - Born 10 August 1913
   - Death registered March 1983, South Glamorgan (Vol 28, Page 3029)

Please advise what records remain available, the correct application
process for deceased patients’ records, any forms or identification
required, applicable fees, and expected timescales. I am happy to provide
further details and proof of relationship/interest as needed.
